RESUMO

Objective To investigate the clincial value of ELISA combined with colloidal gold immunochromatography in the detection of anti-cyclic citrullinated peptide (CCP) antibody in patients with rheumatoid arthritis (RA).Methods From October 2014 to October 2016,120 patients with RA who admitted to Fuyang People 's Hospital were selected as RA group,and 120 healthy subjects were selected as the control group.Serum samples of patients were collected and tested for anti-CCP antibody by ELISA and ELISA combined with colloidal gold immunochromatography.Results ELISA method was lower than ELISA combined with colloidal gold immunochromatography test (x2 =3.943,P < 0.05).The sensitivity,specificity,positive predictive value,negative predictive value of ELISA combined with colloidal gold immunochromatography assay were 93.33%,91.67%,84.85%,96.49%,respectively,which were higher than single ELISA method (all P < 0.05).Conclusion ELISA combined with colloidal gold imnmunochromatography assay for detection of serum anti-C.CP antibody in patients with RA is effective and can obtain high diagnostic sensitivity and specificity,and it is worthy of promotion.

RESUMO

The parabrachial nucleus (PB)is made up of gray matter around the Pons combination(BC),mainly consisting of glutamatergic,GABAergic and enkephalinergic neurons.PB is connected to hypothalamus and basal forebrain through a network of nerve fibers.Specific lesion of the entire parabrachial complex in animals leads to a deep coma.PB also projects to the non-rapid eye movement(NREM)-related regions including the ven-trolateral preoptic,and receives the projections from the parafa-cial zone.Activation of the GABAergic neurons in parafacial zone can promote NREM sleep,which indicates that PB partici-pates in NREM sleep.Furthermore,the lateral PB is actived when rapid eye movement(REM)sleep is deprived.In conclu-sion,PB participates in regulating wakefulness, NREM and REM sleep.This review summarizes the advances in the roles of PB in sleep-wake regulation.

RESUMO

Objective To explore the clinical curative effect of early enteral nutritional support for acute se-vere organophosphorus pesticide poisoning.Methods 56 cases with acute severe organophosphorus pesticide poison-ing were selected and divided into group A and group B in accordance with the random number table.Both groups were given conventional emergency treatment.Enteral nutrition support with nutrition pump was applied for 30 cases of group A in 48 -72hours,while group B in 72h -5d.The time of consciousness recovery,CHE increase to normal 60%,the incidence rate of intermediate syndrome,gastrointestinal complications of abdominal pain,abdominal disten-sion,vomiting,reflux,diarrhea,VAP,MODS of the two groups of patients were compared.Results The time of con-sciousness recovery (3.6 ±0.8)d in group A was lower than that of group B(4.1 ±0.6)d (t=2.612,P0.05 ).There was no significant difference of gastrointestinal complications between the two groups (P>0.05).Conclusion Early enteral nutritional support is effective and safe for acute severe organophosphorus pesticide poisoning and worthy of promotion.

RESUMO

Objective To investigate effects of different sedation methods on the mechanical ventilation patients with acute exacerbation of chronic obstructive pulmonary disease (AECOPD).Methods A total of 101 patients with mechanical ventilation in our hospital was randomly divided into observation (58 cases) and control (43 cases) groups.The observation group was given to strengthen psychological care during the day, and the night was given to the patients with AECOPD.The control group was treated with continuous analgesia and sedation, and the daily morning was executed to wake up the plan 1 ~ 2 hours.A set of indexes were observed, including white cell count, C-reactive protein, oxygen index, B-type natriuretic peptide, delirium occurred rate, unexpectedly drawn tube rate, and duration of mechanical ventilation.Results White blood cell number, C reactive protein, and oxygen index group in the observation group were significantly better than the control group [(10.1 ± 2.4) × 109/L vs (11.2 ± 2.8) × 109/L,(40.6 ± 11.6) mg/L vs (45.8 ± 12.2) mg/L, and (285.6 ±45.1) vs (268.2 ±42.6)] (P <0.05).The incidence of delirium rate in the observation group was significantly lower than the control group [30/58(51.7%) vs 31/43(72.1%)] (P <0.05).The mechanical ventilation time in the observation group was significantly shorter than the control group [(8.41 ± 3.96) d vs (10.35 ± 5.57) d] (P < 0.05).There was no significant difference between B type natriuretic peptide and accidental extubation rate (P > 0.05).Conclusions The night program sedation program can significantly improve AECOPD mechanical ventilation in patients with inflammatory index and oxygen index, reduces the incidence of delirium in patients with acute exacerbation of chronic obstructive pulmonary disease, does not increase the accidental extubation rate, and significantly shortens the AECOPD patients with mechanical ventilation duration of mechanical ventilation, as a mechanical ventilation in AECOPD patients who killed one of the options.

Objective To investigate the effects of hyperbaric oxygen (HBO) therapy on interleukin-1 β( IL-1 β) and interleukin-6 ( IL-6 ) levels in patients with moderate or severe traumatic brain injury ( TBI ) and to analyze its therapeutic efficacy.MethodsOne hundred and fifty-six patients with moderate or severe TBI were randomly divided into a control group and an HBO adjunctive therapy group with 78 cases in each.The control group received essential neurosurgical treatment and conventional drug treatment,and the HBO group was given one HBO therapy session in addition.IL-1 β and IL-6 were quantitatively determined using sandwich ELISA,and the patients'scores on the Glasgow coma scale (GCS) were measured before and after treatment.The Glasgow outcome scale (GOS) was used to evaluate in a follow-up 6 months after injury.ResultsThe average IL-1 β and IL-6 levels of the two groups and their average GCS scores were not significantly different before treatment.After treatment,IL-1 βand IL-6 levels decreased significantly and GCS scores increased significantly in both groups,but there were significant inter-group differences in all three measures.Six months after injury the GOS evaluation indicated that significantly more patients had better outcomes in the HBO group than in the control group. Conclusion Conventional treatment combined with HBO therapy can promote neurofunction recovery in TBI patients.The mechanism may be related to down-regulation of serum IL-1 β and IL-6.

Objective To investigate effects of hyperbaric oxygen (HBO) therapy on C-reactive protein (CRP) and tumor necrosis factor-α (TNF-α) levels in patients with moderate or severe traumatic brain injury (TBI)and to analyze its therapeutic efficacy. Methods One hundred and eight patients with moderate or severe TBI were randomly divided into a control group (54 cases) and an HBO adjunctive therapy group (HBO group, 54 cases).Both groups received essential neurosurgical treatment and conventional drug treatment, and the HBO group was given one session of HBO therapy in addition. Serum CRP and TNF-α were detected, and the scores on the Glasgow coma scale (GCS) were measured before and after treatment. CRP was detected by turbidimetric immunoassay and TNF-α using ELISA. Glasgow outcome scale (GOS) scores were evaluated in a follow-up 6 months after injury. Results Average CRP, TNF-α and GCS measurements showed no statistically significant difference between the groups before treatment. After treatment, CRP and TNF-α were significantly lower and GCS scores significantly better in both groups, but patients in the HBO group were, on average, significantly better than the controls on all three measures.Six months later, GOS evaluation gave a significantly larger number of patients with a better prognosis in the HBO group compared with the controls. Conclusion HBO therapy can significantly decrease serum CRP and TNF-α after severe TBI, thus enhancing therapeutic efficacy.

@#Objective To investigate the effects of conductive education with routine rehabilitation on cerebral palsy. Methods 59 caseswere divided into the conductive group (n=38) and the control group (n=21). The conductive group received conductive education with routinerehabilitation and the control group received routine rehabilitation only. They were assessed with the gross motor function, intelligentand activity of daily living (ADL) before and after treatment. Results The improvement of gross motor was not significantly different betweenthese groups (P>0.05), while the intelligence, ADL improved more in the conductive group than in the control group (P=0.001). ConclusionConductive education can promote the development of the children in ADL and intelligent.

RESUMO

Objective To investigate the efficacy of hyperbaric oxygen combined with donepezil in the treat-ment of patients with vascular dementia(VD). Methods Sixty-four patients with VD were randomly divided into a control group(donepezil group,n=32)and a treatment group(hyperbaric oxygen combined with donepezil,n=32).The course of treatment was 12 weeks.All patients were tested using the Mini-Mental State Examination(MMSE)and Hasegawa's dementia scale(HDS)before and after treatment. Results After 12 weeks of treatment,the MMSE scores and HDS scores of patients increased significantly in both the control and treatment groups.There were signifi-cant differences between the control and treatment groups in terms of MMSE and HDS scores.Conclusion Hyperbar-ic oxygen combined with donepezil could significantly improve cognition in VD patients.

BACKGROUND: Studies have confirmed that RhoA/ROCK signaling pathway plays an important role in the process of epithelial-mesenchymal transition. OBJECTIVE: To investigate the effect of Y-27632 (inhibition of rhodopsin kinase of 4-aminopyridines compounds) on the epithelial-mesenchymal transition of rat peritoneal mesothelial cells (RPMCs) induced by transforming growth factor ?1 (TGF-?1). DESIGN,TIME AND SETTING: An animal observational experiment was performed at the Laboratory of Nephrology in the First Affiliated Hospital of Sun Yat-sen University from March 2007 to March 2008. MATERIALS: A total of ten healthy male Sprague Dawley rats were provided by the Animal Experimental Center of Sun Yat-sen University. Y-27632 was the product of Calbiochem Company (German). TGF-?1 was the product of R&D Company (USA). METHODS: Primary RPMCs were cultured in vitro. After synchronization for 24 hours, RPMCs were randomly divided into 4 groups: control group (RPMCs cultured in the DMEM/F12 without serum), TGF-?1 group (RPMCs cultured in the DMEM/F12 without serum, and then added 10 ?g/L TGF-?1), Y-27632 group (RPMCs cultured in the DMEM/F12 without serum, and then added 10 ?g/L Y-27632), TGF-?1 +Y-27632 group (RPMCs cultured in the DMEM/F12 without serum, and then added 10 ?g/L Y-27632. 2 hours later, 10 ?g/L TGF-?1 were added). The cells were collected 48 hours after culture. MAIN OUTCOME MEASURES: The mRNA and protein expression of E-cadherin and ?-Smooth muscle actin (?-SMA) were measured by RT-PCR. The protein expression level of E-cadherin, ?-SMA and vimentin was measured by Western blotting. AIM: To evaluate the therapeutic efficacy of intratracheal instillation of porcine pulmonary surfactant(PPS) in rats with lipopolysaccharides(LPS)-induced early-stage ALI in this study.METHODS: SD rats weighing 200 g-300 g were randomly divided into 4 groups: LPS(1.5 mg?kg-1)+saline,LPS+PPS 100 mg?kg-1,LPS+PPS 150 mg?kg-1,LPS+PPS 200 mg?kg-1.The PaO2 and PaCO2,as well as survival rate of rats were examined for 6 h after the start of PPS-instillation.Then,rats were killed and lungs were immediately removed for lung index(LI) and histological analysis.The bronchoalveolar lavage fluid(BALF) was collected for measurement of total protein(TP) contents,TNF-? level and white blood cell(WBC) numbers.RESULTS: Significantly increased PaO2,reduced mortality rate,decreased total protein and TNF-? contents in BAL,as well as lung index and meliorated histological appearance were observed in three PPS-treated groups compared with group given saline after LPS(P
